If you are requesting an item from Millstein or another ULS Library for Greensburg course reserve, fill out and submit this form.
If you intend to have a personal owned item placed on reserve, fill out the form and then bring, or send the item through campus mail to the library for processing.
Allow at least 1 week of processing time before expected use by students (note in the comment section if item is needed ASAP).
Course reserves are for 1 semester by default. Note in the comments if you would like items to be placed on reserve longer.
